The first wave is slowing if you take the nation as a whole, The large east coast cities near international airports are all showing declining case loads. There are spikes in places where the social distancing has been reduced. How much they will spike, we will have to see. Most respiratory viruses and certainly most Cornavirus are seasonal so that is why they expect a second wave in the fall when cold and flu season hit. We will have to wait and see.
We are much better prepared for it. Doctors know what to look for. Show up in the ER, test positive and they will check your Oxygen saturation and markers of inflammation (D-Dimers, Ferretin, CRP, Procalcitonin, etc), Your OSAT is low and and if those markers are elevated they will admit your ass in a second. Maybe by then we'll save some idea if Colchicine works. Big study at NYU and Montreal to see if Colchicine can reduce inflammation w/o suppressing the immune system. That and we stopped using Hydrocychloroquine and started using Remdesivir. We look for clotting know and start anti-coagulants earlier.
