Medical Should I send post-interview letter of intent for what would be my top school?

Just interviewed at my top choice last week and would 100% send them a letter of intent, but I wanted to know the deal on doing that before a decision is made/without an update as well? Would this be too aggressive to adcoms or would it help?

It will almost certainly neither help nor hurt. These sorts of letters are so over-sent these day that they mean nothing, if indeed they ever meant anything at all. If it helps your anxiety and makes you feel like “you did everything you could” then go for it; if you’ll fret about whether you came off too pushy then don’t.
