2017-18 Pain Fellowship Interview Thread

This forum made possible through the generous support of SDN members, donors, and sponsors. Thank you.
Well I emailed them my ITE score/diploma and that's what she responded.

Oh ok, I haven't initiated any emails to ITE/Diploma, only when programs have requested it I have obviously emailed back...

Fellow applicants and previous applicants,

What are the most important things to find out about a program on interviews? I know this is different for everyone, but what are you specifically looking for in a program and why?

If a program does not send you an IV when they do send out IVS, is that pretty much a rejection?

I had a program request a copy of my medical degree. Is there a way to upload this to eras or do we have to send it to each program individually?

I'm not getting IVs guys and I'm not sure what could be wrong? I'm in PM&R, I'm sure I'm not the first PM&R person to apply to pain. I am female, go to a solid PM&R program, went to a solid MD school, have a solid record in med school and residency, no red flags, have done previous residency in a different field, have some research, lots of extracurriculars, etc. What could possible be wrong? My app was not complete until like 3/23 or something like that so I'm sure that could be a component but other than that what could be going on? I feel like I wasted a bunch of $ for nothing.

I think it's still early. Many places won't look at applications till April, May and June. I know. it's very hard to be patient. :(

I heard the overall number of pain applications has decreased this year, because anesthesia job market is really good. Anyone else heard about that?

Then it makes more sense then that I would get more not less interviews. I don't have a single one. I'm so bummed that I have gotten overlooked by just about every program so far.

I agree that it is very early - how many of the programs above have you applied to?

Dog_luver, I am a similar candidate to you and applied to alot of programs. Thankfully I've had the luck of getting a few IVs. I think these early interview club programs largely have to do with date submitted. I went to ASIPP a year ago and met with some fellowship PDs regarding applying. Long story short, they all echoed "please apply early! The earlier the better!" I went ahead and prepped my application and submitted by Jan 1 fully accepting that there will be somethings that I wouldn't be able to add to my app (chief position, few extra poster submissions). Its annoying because now I have to play catchup and send out updates (never have i had to copy/paste so much, few mis-spelled names too).

My co-resident who matched this year applied in mid feb of 2016 and definitely missed out on alot of the early programs releasing (from last year's thread). He ended up matching though. Don't lose hope :).

I believe it also depends on where you applied. If you look at the list, very few top-tier programs have sent out invites. There is still hope! :)

No one can be certain of why you are not getting interviews as there are so many variables but the biggest reason is because you completed your application only a week ago. Within that time, only 6 programs have sent out interviews. When a program selects to send out interviews, they have already spent numerous hours evaluating a group of submitted applications, compiling them into yes/no/maybe stacks, then reevaluating these groups until they have narrowed it down to the number of interviews they are willing to send out. You were not likely in this group applications that have been reviewed in the last week because you were simply too recently complete. Some programs will send out a 2nd batch of interviews but more commonly, if you have applied to a program that has already sent out interviews, you've probably already missed the boat on that one. The other reason is that most programs are still very anesthesiology heavy. Nothing against PM&R except tradition.

Barring all other red flags or reasons why you wouldn't be offered an interview, I'd expect you to start hearing back in the next month. Let me know if anyone has any other Qs. Long process but worth it in the end. Good luck.

I think at the end of this process (October) we should answer four quick questions: (1) # of programs applied-to (2) # of interviews (3) match yes/no (4) match position on rank-order list

Fellow applicants and previous applicants,

What are the most important things to find out about a program on interviews? I know this is different for everyone, but what are you specifically looking for in a program and why?

In no particular order...
Name
Location
Reputation
Fellowship experience
Future practice desires (academic vs PP)
Research interests

All of us will have our reasons why we are giving some stuff more importance than the others.

Looks like a couple of the programs still have not completed their ERAS registration. Jacobs School of Medicine at Buffalo is one of them. I am unsure if they are still an active fellowship program or not, however.

How many interviews is everyone aiming for?
PM&R Candidate with fellowship at home institution
-Applied to 43 programs - would ideally like to get ~20 interviews and go to as many as my program/budget will allow.

Does anyone know the significance of getting interview invitation weeks after their initial round of invites? Are those pity invites or what...

UC Davis came in at an ungodly 1:30 AM, some dates already full! Dang california time!

Henry Ford only interviewing on 6/17

Does anyone know the significance of getting interview invitation weeks after their initial round of invites? Are those pity invites or what...

The reasons can be many. It depends mostly on when you completed your application. If you were very early with ample time to be reviewed before the first round of interviews were released, then you may be a 2nd tier applicant. If you applied more recently, then it's likely that you just weren't in time to be reviewed along with the first batch of applicants that received interviews. I wouldn't worry too much about 1st or 2nd tier applicant in the application review process. On paper, many applicants can present better or worse than they actually are for whatever reason and believe me, the reasons are usually exceptionally subjective. In the end, an interview is an interview and once you show up for that interview, everyone is on level playing ground again. Likely no one will know (or care) if you received an invitation during the first batch or second batch of invites. Having an interview, regardless of when you were invited, means they are willing to rank you. The interview will determine how high you rank.

Fellow applicants and previous applicants,

What are the most important things to find out about a program on interviews? I know this is different for everyone, but what are you specifically looking for in a program and why?

Yes it's different for everyone - Depends on what you believe your career to be like after you graduate fellowship. Do you want to join a chill practice in a rural setting back home doing bread and butter pain? Do you want to be in PP churning out procedures? Do you want to be the neuromodulation guy at your practice/town? Do you want to do end-of-life cancer pain? Do you want to conduct basic science pain studies? Do you want to get involved with clinical studies? You should tailor your search for a fellowship based on your goals. This is obvious.

For me, I looked at the following:
1. What is the procedural volume and how interventional is the program? - These are two different things. The procedural volume can be high in that you are doing a lot of injections, but the array of interventions may be low. For example, doing 10-20 LESI's, MBB's, TPI's per day is fine but you also want to make sure the complexity is there (gasserion ganglions, trigeminals, ultrasound-guided peripherals, sympathetics, neurolyses, etc.)
2. How many SCS/pumps? - All the residents want to know but all the fellows/attendings hate hearing this question because fellowship is more than just stims/pumps. But you want to be sure that they are getting at least a good number of these so you can feel proficient. And if their numbers are high, they usually have no problem letting you know that they are high. Yes, there are national cadaver courses where you can learn this but there is nothing like learning first hand from an experienced attending in the OR where you can operate together and pick their brain. You also want to be honing basic techniques and making mistakes when you are a fellow, not when you are in PP where time=$$ and your colleagues and RNs are judging you at your own ASC. You also need a relatively high n value of implants before you see complications come through the ED. If your n=5 in terms of total implants/year, it's rare you'll see the 3-5% of postop infections that will need to be dealt with.
3. Where did past fellows go after completing fellowship? - I wanted to be in a highly desirable urban area after graduation and wanted to make sure the program had a good track record in placing fellows in these types of areas. Some programs are more "national" when it comes to grads (eg. Harvard) where as other equally good programs are more "local" (eg. Mayo, Wake). It's easy enough to go "national" from a more "local" program and vice versa but having alumni already established in many of the cities I was looking at is an extremely easy way to get your foot in the door for job interviews and getting the scoop on other practices in those areas. The pain world is teeny tiny and word of mouth can be huge.

Try this if you're having trouble comparing programs. Have a list of 5-10 different CATEGORIES that are important to you. And have a scoring system for each category and jot down the DESCRIPTION of what each score means. Then after you interview at a program, rate them on each category by looking through the descriptions that best match the program. At the end of the interview season, see how they ranked in terms of total score. If there is a category that is more important on the list (eg. GEOGRAPHY CATEGORY because I need to be close to family), then see how they ranked in terms of this category and compare it to your list of total score. See how the rankings looked compared to your 2nd most important category. This was a fun way metric for me to view programs from a more "objective" standpoint. And I was obsessed with evaluating programs and the Match so it was a good way to satisfy my neurotic tendencies.

For example:
CATEGORIES:
1. Interventional volume
2. Geography
3. Reputation
4. Fellow satisfaction
5. Staff/attendings
6. Schedule
7. My "gut feel"
8. Extra credit (for anything that was truly unique)

SCORING:
Reputation
1 - lower tier program
2 - regionally-recognized program (eg. best in the state)
3 - nationally-recognized program
4 - top programs in the country (eg. my uneducated relatives have heard of it)
Fellow satisfaction
1 - Fellows spoke poorly of the program and seemed unhappy/overworked
2 - Fellows were ambivalent
3 - Fellows enjoyed the program but had a few things that could be improved
4 - Fellows loved their program, wouldn't change a thing, would do it all over again if they had to
