Please add/update this if you have any additional information. Thanks for posting this! That certainly narrows the list down for prospective sports medicine fellows (with PM&R backgrounds). I scoured countless fellowship program websites last spring/summer as I started to apply for sports med fellowship. I wish I had this available to narrow my search last year.

I'd recommend any PM&R residents send emails to any primary care sports programs they are interested in (unless they know for sure that PM&R applicants are sincerely considered). I personally sent emails to a few of the programs listed (west coast programs) above asking if they ever interviewed PM&R residents. Unfortunately, I was told by a few that they did not interview PM&R applicants (despite their website saying the PM&R was a satisfactory prerequisite) and a few others that never responded at all (which I assume meant the same).

Would it be difficult for a physiatrist to gain a patient base in a sports medicine practice as opposed to going FM where they have a broader range of patients? I love the idea of PM&R and sports but I'm curious as to what kind of jobs are out there for PM&R sports docs

I think there are 15 now with the addition of Mayo Minneapolis. 11 have only one spot, the other 4 with 2 positions.
 
Emory has 3 spots. 1 PMR, 1 primary care, and 1 "flex" spot.
