Pretty late, but I wanted to get this ball rolling in case others wanted to contribute. I know it was a boost of confidence last year when I saw the high 60 CBSEer matching at one of their top choices.

I also wanted to offer my two cents about my experience throughout the process. I should start by saying that I am only interested in private practice OMFS. As a result, I only applied to 4 year programs. I'm sure there are many 6 year programs that produce great private practice surgeons, I just wanted a more streamlined training. I also applied to very few programs that operate on malignant pathology. So this is what I learned along the way. Take everything with a grain of salt.. these were my perceptions and they are unique to my experiences. Probably the most important advice I can give someone that is about to go on the interview trail is to listen to everything you are hearing carefully, but ultimately you need to form your own opinions about programs.

Externships:
Oklahoma- Overall it was great externship, I did have to stay in a hotel though. Dr. Smith and Tiwana are great and fun to work with. One attending is not, he is very volatile. The largest problem I saw with OU was how hands on the attendings were in the OR. Regardless, think Oklahoma is very balanced and would be a great place to train.
Pro's: dentoalveolar and sedations, attendings (excluding one), optional 1 month rotation working with malignant path
Con's: They have a craniofacial fellow, OR autonomy

Vanderbilt- It seems like Nashville attracts this program's residents more so than the training. The residents are all very likable people. A few of the attendings are a bit awkward. I'm sure this would change with time if you became a resident there. If you want really good dentoalveolar and sedation training this probably isn't the place for you. Excluding Dr. Vega, the attendings are very hands on and slow in the OR. The Vanderbilt Clinic consisted of consult after consult for surgery work-ups or follow-ups, no action going on in there. I didn't get an interview at Vandy so maybe I'm just salty lol
Pro's- Nashville, residents were great
Con's- dentoalveolar and sedations, attendings cut a lot.

Cincinnati- I really wanted to go here, but I matched at my #2. The resident clinic is literally sedation after sedation after sedation. Dr. Krishnan takes their anesthesia training very seriously. They are using LMA's in their clinic. The attendings are hands off in the OR as well.
Pro's- Attendings are personable, their third 3yr residents had already done 400 3rd molar cases each, all hospitals were on one campus so no driving, anesthesia and dentoalveolar
Con's- small resident room, the resident clinic is pretty dated, their VA wasn't that strong but they just hired a new attending that is supposed to turn the VA around, they have to read A LOT

Iowa- This place was very similar to Cincinnati as far as the resident clinic goes. Sedations on sedations on sedations. The attendings and residents were pretty standard. Attendings were hands off.
Pro's- Anesthesia and dentoavleolar
Con's- OMFS doesn't get consulted for a midface trauma unless there is an associated mandible fracture. IMO this hurts their trauma experience. -- EDIT: this is false, (see below) sorry for mistaken information

Those are the only 4 places I externed at. The comments about the attendings being hands off/on were first hand experience, but that was simply my experience during the 1 or 2 weeks I was there. This could vary with the chief or time of the academic year. I will say I externed at Vandy late in the academic year so that shouldn't have been a factor. Oklahoma was relatively early. Now I will go through some of the places I really liked on the interview trail. I won't comment on the attendings participation in surgery because I didn't extern there and all I know is hearsay.

Favorite Interviews:

VCU: Great program for private practice. Seemed pretty country club. OR numbers weren't that impressive for a 3 resident per year program.
Loyola: Another great program for private practice. Their VA was very strong. Each resident graduates with over 250 implants placed. They weren't sedating at their VA though. They do 400 sedations a year (a little low IMO). If they started sedating at the VA they would boost that number easily.
MedStar: This program seemed very similar to Vanderbilt. Good OR numbers, but well below average sedation and dentoalveolar experience. MedStar and Vandy do around ~380 sedations/year/3 residents
Memphis: This program surprised me. Good numbers and had a nice resident clinic, ~600 sedations/year/2 residents, 200 implants/year.
Augusta: Liked the program a lot. Similar to VCU. Pretty chill but gets the job done for private practice.

Programs I didn't care as much for:

Denver: They didn't put a lot of effort into their interview day
Knoxville: I'm pretty private practice oriented and their sedation/dentoalveolar experience wasn't there for me
Vanderbilt: same reasons as above
MUSC: I felt like a few residents were unhappy there. I could have been misreading the situation, just the vibe I got.

Again, just my two cents. Try to form your own opinions at externships and interviews. Overall, there a lot of really good programs out there so just secure that match spot!

Not true. We alternate midface call with plastics and ent where we manage frontal sinus, orbit, NOE, midface fractures. We are virtually always on mandible call because the other services do not have attendings that like to treat mandibles, this applies to OSH transfers as well. Certainly not a knife and gun club but there is definitely enough trauma to feel comfortable doing a neck cutdown.
Just that clarification.

Light trauma doesn’t necessarily correlate with private practice oriented programs lol. Michigan is relatively light trauma compared to some southern programs but strong in ortho/cranio and cancer and send out many grads to fellowship and academics.

applicants look for different things in an omfs program which is why I agree with masterus! There are applicants who want to be at that light trauma/ country club program and others who want a heavy program where they get slammed every night they are on call, others look for in between-- so to each his own lol
same with cancer-- some want a heavy cancer program because that is what they intend on going into, some want a little bit of it, and others want absolutely nothing to do with it.
i'm pretty sure the only thing that all applicants can agree on is net cost (stipend minus med school tuition)
 
Why do people not want cancer? While I get that most people will not be dealing with it, will we not be getting referrals in private practice for biopsies that end up being cancer?
Wouldn't experience with cancer help with explaining the severity of the diagnosis and setting the tone for what is to come?

EDIT: I'm redacting my former comment explaining what cancer usually means at programs. I thought poster above was a student, but he's farther on the path than I am.

This is getting away from what this thread is intended for, but I don’t necessarily think you need to go to a cancer program to tell a patient what their cancer diagnosis means. My home program does a lot of cancer and it consumes quite a bit of the residents time. At the end of the day there is only so much time during residency. One program can’t do everything and do a lot of everything. You just have to try to determine what your interests are and seek out programs that line up with that. Oklahoma is a program that does a good job of dabbling in a little bit of everything. I think that is something Dr. Tiwana has brought to the program.

Any program that does cancer is an outlier in the curriculum of programs. There are much more programs that don't do cancer than those who do so. In general, cancer programs are not as private practice oriented and that's a whole different category. In terms of general OMFS, more trauma = worse call and less sleep. Some people want that and some don't. Also programs tend to be absolutely SO different from each other.

At the end of the day, it really doesn't matter where you go for private practice because CODA has pretty much standarized training these days. I think people will see that as they go further on in their training. It's like how it doesn't matter where you go for dental school. 20 years down the road, your ability to take teeth out and place implants probably levels out due to experience across the board on average. Of course, some people will still be more skilled than others due to natural skill. It only really matters if you are planning to pursue surgical OMFS and stay in the OR for bigger cases. On top of this, tons of "good" programs have terrible residents that end up being worse OMFS than graduates from "bad" progams when they graduate. It's important to try to learn as much as you can.

The CBSE score has almost no known correlation with what kind of OMFS you will become, your clinical skills, how good of a resident you are as well as your ability to interact with patients and succeed in private practice. Where you went for training will not help some of these skills, some people are just that much better with people than others and more business savvy.
