Five years after medical school, i.e. one year of surgery, medicine, or transitional year, and then four years of radiology.
And yes, most radiologists (~75%) do a fellowship. (http://www.ama-assn.org/vapp/freida/career/0,1238,420,00.html) Most fellowships are one year long. About half of neuroradiology fellowships are two years, and some neurointerventionalists do about three years of post residency training.
