Medical Backing Out on Letter of Intent

I know you all hate me. I know I am a bad person. Please try to remember that I am a human, and I make mistakes.

I sent a letter of intent to school X pretty early in the interview season and was accepted early this year. At the end of the cycle, I was offered a very unexpected interview with my state school. I was waitlisted and then accepted. I feel like I need to go to my state school for financial reasons and also to be closer to family. However, I feel horrible about backing out on my letter of intent.

My question is - by backing out of my letter of intent, will I be blacklisted from all residency programs and destroy my chances of matching? If there is any chance of this of course I will go to School X.

Thank you so much for your advice. I really appreciate it. This decision is agonizing.
