do you feel this impacts yr physican patient relationship or not?
what steps would you take to maintain yr responsibilty towards yr patient.
also what part of it is patient perception,somethimes patients dont feel they have the best care even if the most expensive test/medicine is not given...comments/real worl exprience?

HMOs are one of the parasites sucking the life out of medicine. Maximizing profits is their sole purpose, and they will knock anything over to get it, including physician patient relationship.

Your efficiency in the minimal use of letters is testament to your innate corner-cutting cost-saving nature. I'm sure you will make a great HMO employee. Go for it!

You should never work for a bad HMO.

I know a couple of people who work for Kaiser Permanente (in CA) and they are deliriously happy. You know what the expectations are before you sign up, at the same time you do have the support staff and captive referral networks to get the job done.

But Kaiser is one of the few 'real' HMOs in the sense of the original idea. They are insurer and provider in one entity. (Most other 'HMOs' are just cheapskate capitated indemnity plans that hire a contracted provider network to cover their subjects).
