Hi all, as most of us are approaching to their dedicated study period for step 1, I thought I would start a thread to boost our memorization of First Aid. I am not one of those smart folks with good retention and I need some repetition and time to memorize. So if you guys happen to find some unfamiliar words/terms that strike you, just put those words in this thread and someone can add some explanation. How does this sound? So I will start. (Let's stick to First Aid .)

1. my bad, I should have been more specific about it. I meant to ask prophylaxis for M. avium intracellulare --> azythromycin

2. yes, EN is like a inflammatory process of fat tissues. (may be there is a little relevance though...)

Haha, that is not TB. BTW we also need to know the prophylaxis of TB.

Isn't alpha-1 antitrypsin deficiency assoc with emphysema? You might be confusing it with 1alpha-hydroxyase...?

Is VHL associated with congenital hepatic fibrosis? Or am I thinking cysts?

I think congenital hepatic fibrosis is for ARPKD. In VHL you get bilateral renal cell carcinoma.

alpha1-antitrypsin def also cause cirrhosis. Even earlier (first 2 decades) than panacinar emphysema (4 decades).

alpha1-antitrypsin def also cause cirrhosis. Even earlier (first 2 decades) than panacinar emphysema (4 decades).
Yup, the protein is produced in the liver and is misfolded if I remember right and builds up leading to the liver issues. It is also associated with the emphysema stuff too as mentioned as well.

Holiday heart: a large binge of alcohol can lead to supraventricular tachycardias like a-fib in an otherwise healthy heart. Normal rhythm usually comes back within 24 hours. Known as holiday heart because of course it is associated with binge drinking around holidays. See also post step 1 syndrome.

As another note chronic binge drinking can lead to dilated cardiomyopathy which of course can lead to rhythm abnormalities and should be teased out with the history and further work up as the presentation can be similar.

Whats the pathophysiology behind increased ALT , AST and bilirubin in TSS? Is it just liver inflammation?

Also, avoid nitrates (World Q) (MOA: increase CGMP, veno-vasodilator). You are already having a problem ejecting blood out of the heart, due to the septum pushing on the mitral valve. You do not want to decrease the preload (amount of blood filling during diastole), it will just cause more problems. I missed a question on this today.
