Hey all-- can anyone lay out the difference between a PM&R and FM/primary care sports med sports medicine fellowship? From what I've seen a PM&R sports fellowship is more procedural based including interventional spinal procedures under fluoro, whereas FM seems-- well more primary care oriented?

It is advisable to review the curricula provided by the different sports medicine fellowship programs in which you are interested. Feel free to review them and/or contact the fellowship directors for more specific information. In general, PM&R sports medicine fellowships will provide exposure and training to invasive procedures (e.g., spinal procedures) and Family Medicine sports medicine fellowships will be oriented to primary care.

It is also common to farm-out various issues related to sports medicine. For example, clinical psychological issues related to competitive sports are best referred to licensed psychologists who have been trained in clinical psychology, with specialized training related to clinical sport psychology (e.g., psychotherapy related to competitive cognitive state anxiety issues, without medication).

Here are some links to some sports medicine fellowships (although there are many more, see AMSSM for more listings). Many of them provide a short summary of their curricula related to invasive vs. non-invasive expectations and training related to sports medicine.

I am at a Family Med oriented place, however we are extremely procedural - all areas of peripheral extremity diagnostic and interventional ultrasound (TENEX, PRP, steroid injections).

Go where you will be a good fit and where the fellows are competent in the things you wish to do as an attending. Ask for a list of graduates and what they are currently doing.
