Consider moving to a state without income tax, which state pays doctor well while having a good quality of life?

Alaska, Florida, Nevada, South Dakota, Tennessee, Texas, Washington, and Wyoming

For maximizing income your best bet is SD or WY since they are also rural with lower CoL index and will probably pay well. To answer your question you have to define what is a good quality of life? Outdoors? Night life? Cultural experiences? A pool of other quality people to date and eventually marry? Being close to Trump?

By good quality of life I mean easy to get things like fine grocery stores, outdoors, having a regular schedule to raise kids and pay relatively well.
Maybe Washington state is a good bet? West coast weather, pays better than CA and no state tax?

Not sure what your getting at. Quality of life is subjective. For some this being close to family especially while raising kids is the most important factor bar none, for others, having world class museums/ culture/ airports in your backyard is the price they are willing to pay, for others these are non-factors.

Once you have this sorted out you can look at MGMA data and see what the general pay is in each region and cross reference with income tax free states.

If the only focus is money there are plenty of jobs where you can reach seven figures if you are willing to put in the work and live in the remotest places.

To each their own, but I'd say I doubt anyone will look at their careers in 30 years and say their biggest regret was not being in a state with no/ low income tax
