Child focused journals looking to publish a case report

I have been working on a case report on a patient we had on our child unit. The attending was pretty stoked as it was a child transitioning with their medical condition, PCOS. He wants to shoot for the top journals first and then work down from there as he is pretty hopeful and the topic is pretty hot button in our field. Any advice on which journals to search to next for getting published? Any that don't have extravagant fees to get published?
