Can bradycardia and low BP secondary to hypothyroidism cause chronic ischemic microvascular dz due to insufficient cardiac output? I'm a little confused about terminology. Or does ischemic microvascular dz mean that there has to be pathology in the microvasculature such as atherosclerosis or damage from diabetes?

It means the second.

I think your question is can bradycardia and low blood pressure cause chronic cerebral ischemia. The answer is unless there is chronic microvascular disease, it is unliklely that minor bradycardia and low-normal blood pressure is doing anything.

People usually tolerate heart rates in the 30s and 40s well. a BP of 90 is often normal. How low are we talking?
