I am going into second year and am nearly 100% sure I am going to do small animal. I have always preferred to live in cities and was curious if working as a vet in a big city is different than a suburb (professionally, not personally). Does anyone know anyone who has been a vet in say, NYC or Boston or Miami, etc?

It seems that in a larger city there would be less need for "full service" clinics and more opportunity for specialties. A clinic ina small town may have to be mixed animal to serve the clients in that area and may also have to provide after-hours care if there is not an emergency clinic in the area.

In a large city, general practice clinics can refer to an emergency clinic for after hours care, or a specialist for specific procedures (orthopedics, unltrasound, optho, etc).
 
I know that the vets I've worked for in nyc have all mentioned that there is a lot of competition (in Manhattan at least, I'm not counting other boroughs) because there are just a lot of clinics. I think you have to try much harder not to lose clients because if they don't like you, there's another clinic a few blocks away they can go to. Then again, there are a lot more clients in ny, so maybe it's not such an issue.

On a side note, I think Miami is one of the most awful cities I've ever been to...don't go there.
