What are odds of going from TX to Cali?

I want to do my IM residency in Texas and am a native Texan but would like to do a fellowship beyond that in California - particularly San Francisco. I was wondering what are the odds of making that transition? Also would it be possible to do a fellowship in Texas and then work after all the training in California or is this incredibly difficult to pull off? I know getting a fellowship is difficult to beign with, but I wanted to know specifically about Cali (San Fran) and if anyone has pulled it off before.
