Biggest concerns:
Low on volunteering, hoping that low hour but subjects I'm passionate about and can speak to will make a difference
Sub-par MCAT for MD. I know I should be ok for DO, and while I would be fine taking an A at a DO school I really like, my goal is to do a fellowship to become an intensivist (either IM/Pulm+CC, Anesthesia+CC, or EM+CC) and hopefully have some research under my belt for academic leaning residencies. I know this is quite doable as a DO (haha get it) albeit more difficult.

Should I retake MCAT before June, or focus on service and research while I work? I'm inclined to do the latter as I'm not confident I could improve my MCAT significantly without another large investment of time. However, if my chances at MD are poor, I should retake. If ANY MD program is realistic I'm okay with it

Thanks guys! Would love your insight and help
