Understanding the Finances of the US Healthcare System

Would anyone mind sharing some resources (preferably books, but websites are fine) that elucidate the reimbursement systems that operate in the US? I understand the healthcare system as a whole pretty well, but would like more in-depth knowledge of how Medicare, Medicaid, and private practice finances operate.

You might try The Healing of America. It is too old to cover Obamacare.
