scores vs letters

I am rather confused about this whole process

I've got below average step1 scores and average basic science grades. But pretty much honors all the way across my clinical clerkships evals and average board scores on clerkships. All the surgeons I've worked with pretty much said I shouldn't do anything except surgery. I assume the same comments in my dean's letter will go on my rec letter.

I'm taking step 2 early but based on my standardized score experience....I doubt it'll change much.

My question is, where should I consider applying to? I'm looking at programs w/level I trauma centers. Is it okay if I only look at academic programs? I really have no interest in community programs unless they're really strong in teaching.....
