If I took both the SAT and the ACT and didn't do that well on the SAT, must I report that score? Are we obligated to report everything?

Your application requires you to be truthful and complete, so if it asks you if you have taken the SAT you need to answer "yes" and fill in the score.
Believe me, they do not use your SAT score to decide on med school admissions. Don't sweat it. You will have your nicer ACT score right there as well.
