Questions:
1.If you are transitioning from augmentation therapy to basal bolus, do you discontinue the orals?
2.When to start diary, do you continue to do diary?
3.How to adjust oral medication?
4.Instead of adding medication after 3 months, can we titrate doses of orals upward instead?
5.When patient is at 10%, do you intiate only basal insulin? Or do you add orals as well? (For newly diagnosed diabetes patient)

For your sake I hope you're still a med student or starting as pgy 1.

Try AACE diabetes guideline as well. They have nice algorithms.
