Is there any point in doing Captain's Career Course/Squadron Officer School/(Whatever the Navy Equivalent Is) if you make it to O4 without it? Do you just skip it and go to IDE (or try to skate into O5 and go to PDE if anything)?

At some point, skipping your PME may bite you. Some folks don’t care, some care very much.

As far as the army is concerned, CCC is required for CGSC/ILE and CGSC is evolving to play a bigger and bigger role as a discriminator for 05 to 06 for MC
 
Currently CCC is required for O5 and if you do ILE before boards I think 97% of those who did were picked up.

If you’re getting out as an O4 don’t waste your time.

This post is spot on. For the majority (>90%) of military physicians who will leave ASAP at the end of their ADSO, professional military education (CCC, ILE, GSWC, etc) is a complete waste of time.
If you are planning to voluntarily extend past your ADSO or your initial ADSO puts you past > 12 years of service, than you should at least complete CCC to make 0-5. 12 years is the normal timeframe for ITZ promotion to O-5.
