Patient Forms

So Ive been getting spine pain mgmt started at a clinic that has been focused mostly on joints. I am basically starting from scratch. What I find takes a while is checking patients out in the front before moving on to the next patient. IE filling out imaging forms, telling the MA what the schedule etc. I remember having a good checkout form in fellowship, I think, that had everything needed in a very easy circle format to check patients out so my presence wasnt needed. IE, dx codes, billing levels, image ordering, f/u , etc. Does anyone have a form similar to this they wouldnt mind sending me? I will just make one up from scratch but would like to get some ideas.

Also if anyone has any forms they use when seeing patients to streamline things, that would be great too.
