Couldn't find an answer to this, apologies if it's been asked already.

For cardiology fellowship, is it okay to use an old (very strong) research rec used for residency application? This is because research from residency will not be nearly as good as what was done prior.

Not unless you're applying for a research fellowship. You want current letters that speak to your clinical acumen as a resident. If research is that important to you, get a letter from someone with whom you've done research recently. Your previous research work should speak for itself. If you're applying for a clinical position in a program that is research heavy, consider having your previous mentor call the program on your behalf.
