new practice office documents

Hi All,

I'm starting a new practice and am hoping people are willing to share some of their office documents:

• HIPAA Privacy Notice
• Assignment of Benefits & Release of Billing (to include authorization to download medication history & photos of the patient for their chart)
• Your Practice Financial Policy
• Controlled Substance Agreement

I would greatly appreciate any of these, it would be a huge help.

One of the benefits of being your own boss is to build your practice how you want it. This is the first opportunity to do so. I agree with Google, but make sure you modify anything you copy bc we don't all think the same way.
