New MD Applicant (failed DO applicant from the 2018 cycle)

Nontrad with coursework completed during undergrad. Reapplying from a failed DO run 2 cycles ago.

GPA: 3.83, sci GPA (ACOMAS )was 3.81. Newest course work was 2013 (it has been a LONG time!)

1st MCAT (2017): 506
2nd MCAT (2020): 517
Residence: Illinois (downstate most of my life) with ties to East Tennessee

Volunteering: Library delivery, rape crisis center both crisis line and medical advocacy, tutoring (totals to about 400 hours).
Leadership: QAA Scribe America: remediated, trained, etc. I also mentored other leaders in the organization. The program director for startup sharing functional medicine ideals in elders--program implementation and wrote a coaching programS
Research: MINIMAL--I interned at the corporation I worked for prior to pursuing this medical route! The research was 4 full time on lignocellulosis ethanol production. I, did, however also work in an industrial lab on the quality side for 7 years full time.
Clinical: Floor scribe ED/Peds over 2500 hours
Shadowing: 10 hours formally. I am currently doing the online shadowing. I also shadowed several specialists when they visited the ED, but don't know if that is stretching the truth too much.

I just got my MCAT score back, and I was (delighted but) stunned at how well it went. My fiance has a fellowship in Chicago, so, this is what I am thinking for my application list:
