Medical image data organization

I am working with a research group developing a visualization tool for use by radiologists in their research.

I was wondering if there is any standard or convention for how medical images are organized/categorized in a database. For example, some fields might be Group/Affiliation, Study, Patient/Subject, disease or injury, etc.

When a Radiologist produces a data set, what associated information / metadata is included?
