Hello to all,

I'm USUHS 2006 with some prior SWO experience. I was trying to figure out my intern pay but I have a question that perhaps someone on this board can address. According to my LES, I've been in 10 years but only 6 of them have been active duty (6 for AD, 4 USUHS). After graduation, will I be a O-3 over 10 or and O-3 over 6?

I thought that our 4 years at USUHS do not count as active duty time. I believe that when those of us without prior service start internship we we are considered O-3 with 0 years. After we retire (20 years) then the 4 of school will be applied, but for now it's as if they never existed. That would be nice if they did count though. Anyone else with thoughts on this?

You should be 0-3 over 6 for pay purposes. The 4 years gets tacked on when/if you reach your 20 for retirement.
 
I am currently a 3rd year Navy HPSP at Ohio State and my wife is a 2nd year here in optometry school. So. . .she is going to graduate a year later than me. If I do a Navy internship, then we would have to spend a year apart while she finishes optometry school and I would like to avoid that. Does anyone know how I would go about requesting a civilian internship so I can stay in columbus and wait until my wife graduates. Then, after she graduates and after my internship, we could move on to wherever my GMO will be.

As others have already stated, the 4 years at USUHS only count for retirement credit beyond 20 years of service. So, you will be an O-3 over 6.

Request a 1 year deferment as #1 and make sure you put the co-location issue in your personal statement. With the lower numbers, a 1 year delay may not end up being an option. Should that happen, consider what you would be happy with as a Navy internship and rank appropriately. The Navy does place a high priority on co-location.

NavyFP and Tired thanks for the replies! I have a few more questions that I will post separately so they can be answered individually:

I have narrowed my choice of specialty down to three (EM, PM&R, Ophtho), but I don't really feel the pressure to decide on a specialty right now like most of my Med 3 colleagues. This is mainly because I feel like my next few years are fairly predictable:
Med 4: do a couple Navy rotations (Sept. and Oct.), interview, apply for an internship (transitional year) in medicine. With any luck I will get a 1 year deferment for the reason mentioned in the previous post. After Med 4, I will do my Transitional year followed by a 2-3 year GMO tour and then after the GMO tour (depending on whether or not I am happy in the Navy) I will either finish my commitment by doing another GMO tour and leave the Navy to do a civilian residency OR enter a Navy residency.

Is this a reasonable prediction of the next several years of my life? If so, am I right to not be feeling the pressure to decide on my specialty right now? I am supposed to be scheduling my Med 4 year within the next week and I am not sure if it matters that I do my specialties of choice early in Med 4 since I will not be interviewing for those positions.

So in summary my questions for this post are:
1) Should I feel pressure to decide on a specialty right now?
2) Most HPSP students do transitional years for their PGY-1 year right?
3)Should I still schedule my specialties of choice (PM&R, Ophtho, EM) early in Med 4 even though I won't be interviewing for them?

My question is about being granted a deferment to do a civilian residency. My understanding is that you are more likely to be granted a deferment if you are:
1) Top of your class
2) Apply for a deferment in a specialty in which the Navy is undermanned

Like I posted previously, I am interested in PM&R, EM, and Ophtho. My questions are:

1) How do I know what specialties are undermanned in the Navy right now? I imagine that PM&R and EM are in pretty high demand. Is there any way to know for sure?
2) I was looking on http://nshs.med.navy.mil/gme/RESFELCHT.htm and couldn't find any PM&R residencies. Surely the Navy needs PM&R docs, so do they do their residencies as FTOS or NADDS?
3) I am pretty average as far as grades and board scores go, how much impact does this really have?
4) If I were to apply, would I just rank NADDS as #1 on ERAS?

You could not find a residency program in PM & R because the Navy has transitioned to using contracted civilians in that specialty (just like they have with Rad Onc). I can't provide a solid reference for this - just word of mouth. I highly recommend contacting your GME coordinator (do you have one?) to ask about this.

Detailing is a "negotiation". Detailers like to show this triangle that has "member's prefs" "needs of the navy" "career benefit" on each corner. Note that they are the ones who determine whats best for your Navy career, so figure that, at most, you have 33% input. All interns essentially are detailed after the match, so they can see who will be training or going flight or dive. After that, you'll all start calling and asking for what you want.

Not since 2003
The largest number of GMO tours are with the Marines at Lejune and Camp Pendleton, there are 25 or so on ships and about 75 flight surg billets/year.

Most Navy GMOs are not clinic billets (for first tour GMO's). You won't work 18 hrs days unless deployed.

For example, we could offer that she will work as an optometrist for the Navy as long as they send us to San Diego (or wherever we really want to go). How true is this?

Not true. If your wife were to JOIN THE NAVY, you would be detailed together, otherwise, no bargaining power at all.

1) I would say no. Based on your plan a TY would fit, then you can make your choice. The Navy is not training in PM&R (even deferrals), so if that is your final decision, you will do it as a civilian.
2) No, but there are plenty of TY positions.
3) Yes, while at the hospitals, interview with the Transitional Program Directors.

Just to add my 2 cents to what has already been posted.

4) No, I don't think that will give you any additional bargaining power, unless your wife actually joins. Detailers have no control over civilian hires, so the promise is in a sense empty. Co-location is important to the Navy, so should she come on AD, it would help.
5) Almost all GMO tours are operational. The few that are in clinics are in serious danger of being gapped or going away all together. The best possiblity to not deploy is to be a GMO on a ship that is going into the yards. Not alot of those billets, but you can ask your detailer. Otherwise, plan to deploy.
