I did my undergrad at a small private University in the UK, there were just over 1,200 students, and the towns’ population was a little over 10,000. I found this small environment very conducive to study, as apart from the library, or the bar there was not much else to do - would you say this is the same for Caribbean school?

While some of the islands have lots to do (beaches, wildlife, nightlife, etc.), med. schools based on islands such as Saba fit a similar description that you describe.
 
sounds like my experience at ross as well.

If you want stuff to do at any place, you'll find it. But its harder at some places. Dominica was one of those places.

Definatly no "movies" to go out to even.
