International Medicine without an internship

I recently met an american trained FP that spent 1 year practicing medicine in central america right after graduating from medical school. He then returned and entered the match and completed a residency. Is this still possible? Has anyone else out there done something similar? It's very interesting to me and would love to find out more about such opportunities. Any leads? Thanks!
