I am applying for this may 2024. I am 23 y.o; I had an IA in my senior year of college in 2023. That semester was challenging as my fathers declined and I was his home health aide. I had to commute back and fourth resulting in me making the poor decision. I have since been trying to redeem myself but the clinical informatics specialist/medical scribe position I am working for is really not a good fit for me. I have 560 hours working 45 hours a week in 3 months. It is my most recent and meaningful experience but I feel I got all I wanted from the job, physician LOR + shadowing + most meaningful experience. I have concerns, with all the things wrong with my application, that quitting will make me look bad. But I do need to get my nonclinical volunteering up to in my application so will it be too bad of a decision if I focused on getting my nonclinical volunteering and hospice twice a week up until may what else should i do?

More Context:

Institutional Action Write Up (over word limit):

During December 2022, my first semester of senior year, an uncharacteristic aberration occurred in one course. I wound up relying too heavily on a colleague's lab report, implying it was my own—an action I now recognize as unacceptable. Contributing to this lapse in judgment was the heightened stress stemming from my father's hospitalization, which necessitated my transition into the role of his home health aide, alongside increased family responsibilities. I regret not asking for an extension instead. Asking for help before it is too late is a lesson I have carried in my professional and personal life. Prior to this incident, I consistently upheld academic integrity. Knowing what I did was wrong, I plead guilty to the subsequent student conduct investigation that resulted in significant consequences, including a 0 for the lab report and a final course letter grade reduction. Since, I actively engaged in self-reflection and mandated integrity-related activities which expunged the case from my transcript, underlining my commitment to personal growth. Despite this I returned to baseline the next semester a testament to my resilience and continued learning. Asking for help before it is too late is a lesson I have carried in my professional and personal life.

I am no longer going to be working there, if i go through with it, on february 27. Then I will have 3 months until primary apps. I have hospice volunteering lined up which is clinical volunteering but that is 2 times a week. I am also lacking some nonclinical volunteering which I could do until may.

Why is this position not a good fit for you?

I agree you should wait until you get sufficient experience before applying. More food bank hours. Maybe instead of home aide go to hospice if your can. Find your purpose as a healthcare provider.

How is your IA documented in the student conduct office?

Actually I have hospice volunteering lined up I already did training but the process should go faster depending on health clearance. I loved the patient interactions I have gained from working as a CIS and it may be one of my most meaningful experience. I got to use my language skills in helping the medical assistant conduct tests such as SLUMS to eliminate language barriers that can lead to false documentation. It is the pay for the amount of work I do. I rather work in a higher paying research position, if possible, for paid employment.

If your IA shows up anywhere in your college records it may limit your chances for interviews. Apply broadly to MD and DO schools and I suggest these:
All 4 SUNYS
Albany
Vermont
Quinnipiac
Hackensack
Penn State
Drexel
Temple
George Washington
Virginia Commonwealth
Eastern Virginia
Wake Forest
NOVA MD
Belmont
Rosalind Franklin
Medical College Wisconsin
Oakland Beaumont
Alice Walton (when it opens)
For DO schools I suggest these:
Touro-NY
LECOM (all schools)
NYITCOM
UNECOM
PCOM (all schools)
WVSOM
UP-KYCOM
CUSOM
MU-COM
DMU-COM
ATSU-KCOM
CCOM
AZCOM
TUNCOM
UIWSOM
ACOM
WCU-COM

This is what I found on reddit from a student also from sbu but i do not know how valid it is;
This happened to me for forgetting my phone in my pocket. My school said that I never have to disclose it unless i get caught again and then it goes on my transcript :O if its not on your transcript you dont have to disclose it. I go to stony brook university, finishing my undergrad this semester. They said thst no school will know about it UNLESS i go to their medical school (Renaissance School Of Medicine) since they all share the same administrative system. I have no plans on going to school there anyway. DO NOT DISCLOSE IT. If it is not on your transcript then it was not an egregious enough offense.

Any advice would be great

What does your activity of Clinical Informatics Specialist involve ? Any patient contact ? What did you do as a Home Health Aid and how many hours ?
Yes patient contact ! Going into the room with the physician and scribing the visit while also promoting the provider to do the monofilament exam, prompt for vaccinations and if the patient says they already had it but there was no documentation in CIR prompt for neccsary covid, pna, and flu vaccines. The most memorable interaction was when I was able to directly use my language skills to assist the talented medical assistant Mr.Fritz in translating SLUMS which is a questionnaire to screen for potential dementia in bengali on “Mr.Patel.” Giving patients necessary referrals to see a sigh of relief ( a patient had active hep c was reffered for treatment but did not follow through medications given and i handed him the referral for ID (infectious disease) and he actually followed up as I saw him again scribing the next progress visit. or offering words of encouragement in grave situations with multiple diseases from uncontrolled diabetes, ESRD on hemodialysis, leg amputee, prostate cancer patients. As for the home health aide it was November 2022-Jan 2023 (gave food through peg tube, administered medication, feeding, changing catheters, bathing, wash clothes and folded it, took him to physical therapist appointments, making appointments and following up with specialists 2500 hours)

IA's should never be placed on a transcript, but documentation can be requested in a "dean's certification letter" after you have been accepted as a condition of matriculation.

Pay close attention to the prompts on whether you should disclose. Work with your student conduct office to be sure. (Anyone who advises on this topic with "Absolutes" often gets others in real trouble.)
