How can one stay away from being out of practice

Hi
I am an IMG and I am in Canada right now planning to wright the MCCEE, MCCQE1 and TOEFL. This may take long time. Currently, I am out of practice for 4 months.
My question is; How can I stay away of being out of practice till i do all the above mentioned exams? Because being out of practice for a complete 1 year ( In some provinces 2 years ) may makes me unable to apply for residency/practicing family medicine.
are their any ideas? does electives/observeship keep me away from out of practice trap?
