General Surg to ORTHO?

Are all Ortho surg programs fast track programs, directly out of med school programs? Are there any programs where u can apply to after doing a general surgery residency? If someone where to do a year or two in general surg could they switch into a ortho surg spot (in the same institution or another institution)?
