I am still unclear in my search for a specialty i have long wanted to be a Maternal fetalmedicine specialist. I have only resently started to contemplate orthopaedic surgery with a fellowship in either hip and joint or spine. What are the benifits of each specialty in terms of compensation lifestyle and difficulty to obtain? Thanks

This discussion about ortho specialities is pretty good.

I could not tell you about fellowship competition, but getting into ortho itself is difficult. Most residents are top 10-15% of their class (GPA and rank), have high USMLE scores (240), and honors in many of their clinicals. Many have grad degrees and have been published.
