Can anyone comment on the strengths/weaknesses of the CV and ICU fellowships in CA and NYC? In particular, what are the views of Columbia, UCLA/SF/SD and Stanford?

Don't know about the others but I can comment on UCSD.
No CCM fellowship available.
Cardiac is so-so. You will do a lot of pulmonary thromboendarterectomies on patients with pulmonary=systemic pressures. All these cases use echo+circ arrest. Skills are transferable to private practice. There is not much volume in coronary revasc and valves.
