when do you decide between going into surgical or medical. is it sometime during med school, or when you apply for internship?

You select your future path during med school. Intern years are very different depending on the program you're going into.
 
A medical student usually decides whether he or she wants to enter a surgical specialty or a medical specialty during medical school. When you apply for internship, which is commonly called "residency," you can apply to either medical or surgical internship or to both.

Med Schools the same for both.
Residency after medical school is different and the length depends on the specialty.
7 years for general Surgery to attain attending status
while Family or Emergency medicine for example is only 3 years.

Good website.

I found out that an opthalmology residency lasts four years.

Most general surgery residencies are a minimum of 5 years in length.

There are some programs that require an additional 2 years in the research lab (for a total of 7), some residents who choose to do additional time in the lab (usually 2 years, sometimes more or less) and of course, fellowships add time on. But you can become a surgical attending in 5 years of residency, although most academic surgeons will have spent time in the lab as well as have done a fellowship.
