From the CARMS website:
"ANNOUNCEMENT!!! International Medical Graduates who meet the eligiblity criteria set out by The Council of Ontario Faculties of Medicine (COFM) are eligible to apply to second iteration positions in Ontario through CaRMS"
Basically, it seems that you must be a Canadian Citizen or have Canadian PR and have graduated within the past 3 years from a medical school on the FAIMER list.
