can u pass cs w/o documenting some of ur physical exam findings?

can u pass cs w/o joting down on the computer/note sheet some of ur physical exam findings? i.e. u did quick check of reflexes but forgot to note it. or u did jvd but forgot to note it (it was normal). i realized after exam that i'd asked some questions/did some PE tests that forgot to record...arggh.
