So I'm heading to Wright-Patterson this weekend and I'm working on getting everything together to go. I was reading the AMP 101 Welcome Letter that got sent to me yesterday. It says on the first day of classes we need "copy of your orders, 1042 and physical, and DOD IA Form."

Ok I got some of that...orders, check. I did some internet digging and found that he 1042 is a downloadable form to clear us for flying. But physical? Do we need to have documentation of a recent physical and if so I'm assuming it needs to be from a physical approved by the USAF? And the DOD IA. Any ideas on what that is?

Sorry if these seem like silly questions. I'm still figuring out form numbers and everything like that. Any help would be greatly appreciated!

Do you have an initial flying class physical (IFC)? You should if you're going to AMP. Are you at a base right now? Have you gotten an annual physical (PHA) anytime recently? If so, just go by the Flight Medicine office and have them print you up your most recent 1042. You can't make a 1042 yourself by downloading it off the net. Sorry. But if you've had a initial flying class physical done recently, that will do in a pinch. Just take your copy of the IFC (and any waivers you have for bad vision, etc) with you and you'll be fine. Don't worry about the 1042.

The DOD IA is short for Information Assurance. It's the DOD computer-based training thing that you have to click through to use military computers. Tells you not to use thumb drives or download pornography onto government computers. The AMP people want this from you because then they can let you use the schoolhouse computers.

Don't panic if you can't pull all this stuff together before you get down there. They'll help.

Thanks, AF M4!

The last physical I had was sometime last year when I was applying to USUHS. I also have my MEPS physical too...I'm not sure if either of those includes initial flying class information. I'm not at a base right now so that makes this a bit difficult. I think I'm going to try calling the AMP coordinator office again in the morning to see exactly what they're asking for.

Do you know if there's a site to take the IA course? I seem to remember taking something like that at COT last year, but all the documentation from that was kept there at Maxwell.

Thanks again for your help. I was sort of in minor panic mode. It's good to know there'll be some help if I need it once I'm there. I kept imagining scenarios from COT where there'd be lots of yelling for simple stuff.

Ohhh...I gotcha. Sorry, didn't realize you were currently in med school. You're in USUHS right now? This is the summer AMP 101 thing where they bring you up for about 2 weeks right?

You really don't have much to worry about then. Most people don't get their first 1042 until they've gotten to their first active duty assignment as a flight surgeon.

What's happened here is that they give AMP 101 to med students, reservists, active duty docs who are already at their bases, etc. And so they just spam out the same requirements form to everyone without taking into account that if you're a med student then you're just not going to have had an IFC or 1042 done. Don't worry, you're not deficient.

Oh, and AMP 101 isn't COT. Not at all. This is a two-week sales pitch to get you to want to be a flight surgeon and no one's going to yell at you about anything, ever. Have fun and relax.

Try googling the ADLS (Advanced Distance Learning site) and see if that will let you in. Failing that, yeah, ask the AMP Daddy (or Mommy) how to get the IA and if you can just do it when you get there. You probably can; the tech guys there will generally let people log on to get that stuff done.

Awesome, thanks again! I'm actually at Tulane, not USUHS...I applied to USUHS but went with HPSP instead haha. ADLS sounds familiar, so I'll definitely try to find that.

Thanks again, crisis averted!

They will tell you if you need it or not. In the old AMP (pre 2010), we went to graduation right after getting off the bus from SERE, without even showering or anything. So obviously we didn't need anything than our old ratty ABU/BDU which we wore in the field. No more SERE there, so you'll probably be in blues or something. Again, if you need the dress coat they will (probably) tell you.
