Getting an acceptance is on you, but you shouldn't need to apply to DO unless you really want to avoid becoming a reapplicant and repeat everything and spending more.
If costs of applying are an issue, then I don't get why you are applying far away from the East Coast. Why do you feel you are a mission fit with UCSF? Wayne State? Albany? Do you have enough volunteering hours for the Catholic schools? A different service orientation focused activity other that does not clinical or tutoring will strengthen your application (food distribution, shelter work, housing rehabilitation)... I don't think you need that much since you do have work with seniors and the incarcerated, but you have a lot where you are tutoring ("one-trick applicant?").
