Does anyone know what the letter of recommendation formats were like for JABSOM? Does JABSOM require only a letter from recommenders or does the school require a letter plus some kind of completion of a form where you rate the person numerically on various traits?
Also what is the timeline for when you know you need a recommendation to when they are actually due?

There is a LOR form that you can request from the school or you can just have the recommender send a letter. LORs are due 2 weeks after receiving "Packet 2" which includes the invitation for interviews. Here are the questions from the JABSOM LOR form:
1) How long have you known this applicant and in what capacity? Is the applicant related to you? How frequent is/was your interaction with the applicant?
2) What are the applicant's strengths, particularly as it might apply to the study and practice of medicine?
3) What are the applicant's weaknesses, particularly as it might apply to the study and practice of medicine?
4) Would you wish to have this applicant as your physician when he or she completes medical training?
❏ Yes ❏ No
and Why?
Please comment on the following attributes of the applicant:
Communication (ability to express ideas and opinions orally and in writing; responses to questions; verbal facility)
Compassion (desire to help people; empathy)
Leadership (successful leadership experiences; evidence of influencing others in pertinent to background, e.g., sports, church, clubs, service organizations)
Maturity (appropriate maturity for age; ability to handle responsibilities of medicine)
Motivation (derivation and strength of desire for medical career; career perspectives)
Self-Appraisal (How well does the individual know himself/herself?)
Support (Does the applicant have a support system that he/she can rely upon in times of personal need?)
Teamwork (Can the applicant work with others? Is he/she a loner?)
Additional Comments:

I couldn't find anything about UHM but UH Hilo has some explanation of how to establish residency. If UH Manoa/JABSOM has similar requirements to UH Hilo, then the following link may help:
