some opinions re: how well they work for 1st MPJ arthritis and how painful they are would be appreciated.

Some studies suggest they are no more effective than saline injections. But, they are low risk and worth a try. Now, have whoever has thumb pain see their physician for medical advice.
 
US guided in the office, not very painful since no bouncing around trying to get it in. Worth a try.
