Extremely variable between programs and between residency years. In my residency, the PGY-3 year is the toughest in terms of night call and we had to do an overhaul of the call system to comply with the 80hr rule. Also, regardless of the hours, radiology call is in many programs very very busy. Almost all of my fellow classmates who did surgery internships in hardcore general surgery programs thought their internship was much easier than their PGY-3 radiology call (and their internships were before the 80hr rule). And consider that you are doing this much call in your PGY3 and 4 years, not as an intern, which makes it harder.
But again, this is very variable between programs and I am sure there are cush programs out there as well.
