Question I got wrong from the main CK q bank about TB pt with pleural effusion asking what you gonna find in the thoracentesis fluid and I said empyema (i.e. neutrophil predominant exudate, much like in bacterial pneumonia) but answer was lymphocytic exudate. Looked up the literature looks like it can be neutrophilic predominant later on in disease but starts off as lymphocytic somehow by a mechanism involving hypersensitivity that I don't understand.
Is this unfair to have to pick between empyema vs lymphocytic exudate for TB pleurisy?
