Arkansas is a great place for private practice dentistry. Very few dentists in the state (no dental school there), very few if any HMOs, Very easy to start dental practice due to huge need and low overhead, also easy to run a fee for service practice. Most private practice dentists do really well and only work Monday - Thursday pretty much anywhere in the state including large cities
Other areas in the south are probably similar.

Conventional wisdom would suggest that the door is a great place to open the practice. I have discovered that patients find entering through the window to be off-putting. Hope that totally helps! Incidentally, were the h's a sort of breathy indication that you were whispering?
 
How about Alaska or Puerto Rico? Both are vacation spots.
