What if you have no home program?

My school has a small ENT department through which a few student rotate through during surgery, but no real research department or a residency program.

What kind of things might be recommended to overcome this? Specifically as is related to all the talk about aways; perhaps I should aim to do aways at some middle tier programs in hopes of obtaining a letter from someone than my program's chair?

All other suggestions are welcome.

Rotate at your home institution as soon as you can. Do two away rotations.

Get a letter from someone at your school who knows you well and can write a positive LOR. Get a good chairman's letter early (1st away rotation). Add another letter or two from your second away rotation.

Ace the boards. Get involved with ENT research as soon as possible.

It's definitely possible to match in your situation but it will be tough.
 
People understand if you don't have a home program. My school lacks a program as well and even though we are a small school, we get 1-2 folks a year into ENT. Do some aways and impress them and do well on the boards.
