What does case by case mean on MSAR?

I was looking at some schools and noticed that it said "case by case"/

Obviously, it means that it depends but what does it depend on? Stats?

It usually means that it is an uncommon occurrence.

Sorry to dig this up again, but do you mean it is an uncommon occurrence for the school to accept the credit, or uncommon for people to have that issue? I took online biochemistry and a lot of schools have case-by-case for that I've noticed.
