USA Today Article: Inside a pharmacy where a fatal error occurred

I'm new here, but I feel I had to jump in. I am not a pharmacist or a pharmacy student...yet. But I am a nurse with public health and methadone clinic experience...so I'll give my two cents. This is a horrible tragedy all around and there are many that could be blamed. However, patient safety week just passed not too long ago and one of the focuses was getting patients involved in their own healthcare. I, for one, always check the prescription bottle when I get home to make sure it matches what I was told by the Dr. Patients have to get involved in their own healthcare; no one is perfect and mistakes happen, unfortunately. And yeah, who the heck takes that many pills without questioning it? 80mg in my experience isn't that much, but of course I was dosing those that could handle a lot more. Just my two cents.
