Why is there an amount enclosed line on the form? Are you supposed to send money with it I read the handbook and it didn't say anything like that.

Depends where you are sending the request form to. It costs money to get an official transcript from some schools. Ask your school(s) registrar's office how much an official transcript costs, and if they need anything else from you.
