In general, do you need very good undergrad grades to get in a surgery subspecialty like ortho or neuro when applying to residency, or do they only care how you perform in medical school?

No and yes. No you don't need them, yes they might get you into a better med school which may increase chances of marching.

My 2¢ from reading the forums.
 
No one even has access to undergrad grades AFAIK
