I'm confused between how the above presentations differ.

Is the following correct?

Carcinoma head of pancreas - Jaundice + Pancreatitis + Non tender GB swelling (sometimes/always?)

Stone in ampulla of vater - Jaundice + Fever + Pain (Charcot triad, no gb swelling ) + Pancreatitis

It really depends on the size of the tumor and the individual anatomy. Obviously if the pancreatic tumor obstructs the cystic duct or the common bile duct, there will be cholestasis and the chance of inflammation. Even in the presence of all this, depends on when you catch it early vs late for GB swelling to be noticeable on ultrasound.

Stone in ampulla of vater: Acute presentation with Jaundice, Fever & Pain

Carcinoma head of pancreas: Insidious onset with painless Jaundice +/- palpable GB
